Pathophysiology[edit | edit source]

The pathophysiology of cerebral arteriosclerosis is almost similar to the arteriosclerosis in other regions of the vascular system with minor differences. [1]

It is a gradually progressing disease, initiated by the stiffening of cerebral blood vessels due to increased collagen and fibrosis in tunica media and tunica intima majorly due to aging. This combined with chronic injury to endothelium due to conventional risk factors like hypertension, hyperlipidemia, smoking will lead to endothelial dysfunction, macrophage activation, smooth muscle proliferation. All these pathological processes cause the accumulation of lipids and the formation of atherosclerotic plaques. [2] The slow expansion of these plaques or the occasional rapid expansion due to thrombus aggregate from small plaque rupture can lead to occlusion of these vessels and obstructs the free flow of blood to the brain. Large cerebral vessel occlusion by these plaques can result in stroke and small-vessel occlusion can cause vascular dementia. [3]
