| Does the patient meet ANY of the following criteria? | Hospitalized > 96 hours |
| Body weight < 47 kg | |
| Unable to receive nourishment (oral, NG) | |
| Life expectancy < 8 weeks | |
| The patient does not meet any of the listed criteria |
| Does the patient meet ANY of the following criteria of high bleeding risk? | Significant GI, pulmonary, or GU bleeding, chronic peptic ulcer disease or gastritis |
| Clinically significant bleeding within 6 months | |
| Any IC bleeding, cancer, trauma or lesions within 3 months | |
| Received fibrinolysis or thrombolysis in prior 30 days | |
| The patient does not meet any of the listed criteria |
