The majority of children with headaches have a primary headache disorder^[1] or secondary headache to a self limiting condition.^[2] However, a number of patients have a serious secondary headache.^[1]

• Red flag features of serious headaches in children include age younger than 3 years old, recent onset and worsening pattern, early morning awakening with headache or vomiting, headaches worse with straining, seizures, mood change, mental status change, change in school performance, cafe-au-lait macules, and hypopigmented macules.^[3]
